CQI OBRA REGULATION TOOLS** - CHAPTER III - All of the OBRA regulations are set up as continuous quality Improvement (CQI) audit
tools that can be used to perform ongoing reviews and mock surveys. These tools also serve as a quick reference for staff on best practice
and regulation requirements. Administrators and DONS new to LTC find this manual invaluable for learning the regulations. These tools
serve as a map to clinical staff for reviewing the new requirements for pressure ulcer prevention and care, urinary incontinence, urinary
tract infections, Medical Director Requirements, unnecessary medications and pharmacy regulations. All of the clinical areas under the
quality of care tag numbers are included in the CQI tools. A new department head can use these tools to learn expectations for their
department. For example there are four CQI tools for the Dietary Department - dietary assessment, care planning and documentation,
dining/meal service and dietary department requirements. Additional tools to assist department heads are: Environmental, Safety and
Life Safety; Activities, Social Services, Nursing department programs; Restorative Nursing; Infection Control; OSHA; Orientation;
In-service; Abuse Prevention; Personnel Files; Resident Rights and Dignity. The Administrative CQI tool serves as a checklist for
administrative areas that need to be current and in place. There are a total of sixty-five CQI tools in the manual.
** The latest regulations and investigative protocols are included. as of the April, 2006 3rd. edition completion date. Ongoing updates to
regulations will be made available yearly as applicable.
QI/QM ANALYSIS TOOLS - CHAPTER III:
Each QI/QM is formatted to be an analysis working tool and includes the involved MDS items,
risk adjustments, exclusions, numerator, denominator and technical specifications. The tools guide the reviewer to conclude if the triggered
QI/QM was due to inaccurate MDS coding, deficient clinical practice or case mix. Use of these tools helps a facility to identify exactly
what their triggered QI/QM percentages mean and proactively address them. Chapter I addresses how to use these tools.
CQI ASSISTED LIVING TOOLS - CHAPTER III:
Regulations vary from state to state. This section includes 20 CQI tools specific to
assisted living.
